What should I bring to my office appointment?

You have the option to print and complete your registration forms in advance and bring them to your appointment:

Patient Registration Forms

Depending on the nature of your appointment, you may want to bring:

  • Patient registration form (see link above)
  • Health questionnaire
  • Medication list
  • A list of your past medical and surgical history
  • Digital copies (not just reports) of previous radiographic imaging (CT, MRI, Ultrasound or X-Rays)
  • Drivers license or state issued identification card
  • Insurance cards
  • A list of questions you may have
  • A friend or family member

 

How should I prepare prior to surgery?

Specific instructions pertaining to your surgery will be given to you by our office personnel. However, here are some general tips for preparing for surgery:

  • Hold all blood thinners or anti-inflammatory medications 7 days prior to your surgery, unless advised otherwise. These include Aspirin, Plavix, Coumadin, Motrin, Ibuprofen, Advil or Aleve. Patients on Xarelto or Lovenox will be given specific instructions on when to hold such¬†medications.
  • The night prior to your surgery, nothing to eat or drink after midnight. If you have to take medications, a small sip of water is OK.
  • Hold all blood pressure medications the morning of surgery, except for beta-blockers (Metoprolol/Lopressor, Propranolol, Atenolol)
  • If you have any questions, call the office before the end of business day.
